Oakwood Care and Rehabilitation ranked as the second largest nursing home in Jefferson County for the first quarter of 2026, the Centers for Medicare and Medicaid Services (CMS) reported.

The facility offered 170 beds—70 more than the Jefferson County average for the period—and supported an average of 118.6 residents per day.

The CMS assigned Oakwood Care and Rehabilitation an overall rating of 1 out of 5, falling short of the Colorado nursing home average of 3.2.

Within the same quarter, The Lodge at Red Rocks was the largest nursing home in the county with 180 beds, while Mountain Vista Health Center was third largest with 168 beds.

Through the first quarter of 2026, Oakwood Care and Rehabilitation had accumulated $235,923 in fines across four violations and faced six penalties.

As of that period, there were 21 for-profit nursing homes operating in Jefferson County, Colorado.

U.S. Census Bureau estimates show that the nationwide population of older adults continues to increase while younger age groups decline. From 2020 to 2024, the 65-and-older group grew by 13% and the population under 18 decreased by 1.7%. The nation’s older adult population has expanded significantly over the last 100 years, going from 4.9 million in 1920 to more than 61 million by 2024.

A 2021 study finds that almost half of people in this age group—an estimated 28 million—may need long-term care and support. More than one-third are projected to eventually require nursing home care.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an insurance education organization. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
